All co-pays are due prior to the service. We accept cash, checks, and debit/credit cards (Visa/MasterCard/Discover).

You should bring the following:

• Valid Driver's License/other form of identification
• Current insurance card(s) or lawyer information
• Referral from your doctor (if applicable)
• List of current medications
• Any past diagnostic imaging/radiology reports

The more information we have on your health and medical history, the better we can serve you! 

If you are getting an EMG/NCV test performed, please do not apply lotion, oil, or any other moisturizing products to your body prior to your appointment.
